If a single equipment item is over $5,000 it is considered an asset. We need additional information to place
equipment orders. If the order request is over $10,000 please provide two additional bids (3 total) or a completed bid wavier form when two additional
bids are not available. Also please provide the answers to the questions below. Please contact PEFS-Purchasing
if you have any additional questions.

5) Are any of the items on the quote considered consumables? Items that will be used during a process that have less than 1 year life expectancy?

Additional items to consider when ordering equipment.
1) Will this equipment require any changes in the lab? Any special electrical or structural changes required?
2) Will the delivery to you lab require special handling? Is the size of the equipment to large/heavy to delivery in the elevator, hallways or the doorway of the lab?
3) Is the company including inside delivery or will this require a third party vendor to deliver from the loading dock to the lab?
